The Editors intend to speedily
publish peer-accepted eLetters that contribute substantially to
the topic under discussion. Accepted eLetters will often be published
within ten days. Prior to publication, eLetter authors must complete
a conflict of interest form. Authors of eLetters must reveal their
name and e-mail address. Anonymity will be granted only under exceptional
circumstances at the discretion of the Editors.
